Transition Diary - Dear Diary, I am overwhelmed...
Dear Diary - I haven't a clue. So, I am blogging, right now as best I've worked it out, then I'll catch up on the bits that I remember over the next few weeks and then I'll blog in real time about moving from children's services to adult services - aka "TRANSITIONS"
Dear Diary, I watched this video from "SENlegal" and I realised, I have to get my act together

Right now I am feeling overwhelmed.
Our movement from children's services to adult services seems confused and interrupted by Covid and crazy disorganised.
I reckon that there are bout 4 areas I meed to be thinking about -
and it all seems to fall on me because we don't have a social worker support because there are no safeguarding concerns and that's they way it works here in Haringey and this is what they've told me.
I have listed the 4 broad areas that I think that I need to think about in the picture below.
I reckon that I need to think about stuff about his state benefits and support like moving from 'Disability Living Allowance' to a 'Personal Independent Payment'. Some of this has happened already and I'll write up about that next.
Then there is the support from Haringey SEND and the maintenance of the Education, Health and Care Plan (EHCP).
That does not seem to have happened as it should have done in accordance with whats in the Transition's Policy.
We are having more medical appointments right now.
Partly because the lockdown ended and the NHS is 'open' again and partly because the paediatric (children's) services are gearing up for a a handover to the adult doctors.
And then there is all the stuff about Mental Capacity,
Power of Attorney and Deputyships. I have no idea where to start with that, so I have attended a seminar by some lawyers (who also so YouTube videos) and have arranged an appointment.
